Information you choose to share

The availability form collects your role, name, phone, email, who needs housing, room preference, move-in timeframe, prospective resident’s age, preferred contact method, and any optional message. Share someone else’s information only with their permission.

How inquiries are used

Resident applications and professional referrals open in Jotform and are submitted to Main Haven through that service. Those forms collect the information described on each form, which may include supporting documents, acknowledgments, and signatures. Jotform has its own privacy practices.

Availability inquiries are stored for Main Haven staff to review and respond about housing and next steps. The form is not a medical intake, full resident application, or formal professional referral. Do not include medical records, Social Security numbers, banking details, or confidential case files.

Website services and security

This website uses hosting and data-storage services to receive and store inquiries. Authorized staff access to the inquiry inbox requires sign-in and an approved account. Technical request information may be processed by hosting providers. The form uses a hashed network identifier for abuse prevention; it does not store your raw IP address in the inquiry record.

The site does not add advertising trackers. Fonts may load from Google Fonts. Embedded maps load from Google Maps, which may receive technical information when the map loads. External sites and services have their own privacy practices.

Your choices

You may call instead of using the online form. Contact Main Haven to request a correction or deletion of information you submitted, or to ask about retention. Do not use this form for emergencies or time-critical placement; call us directly.
